How It Works

Our goal is to provide clinicians with maximum value from minimum input. 

 It works as a separate tool from your EMR allowing you the freedom to utilize it when you want and how you want to supplement documentation or demonstrate your patient outcomes.

How it works 

1) Use any tablet, phone or laptop with Wi-Fi to complete the questionnaire.

2) The patient answers  4 questions.   (<2 minutes)

3) The clinician answers 4 questions. (<30 seconds)

4) That's it! After that the data is stored and can be accessed via the dashboard or supply report when needed. Dashboards can be easily printed or  added to a patient's chart as a PDF.

Security

• We take patient and clinic privacy seriously. All data is stored in a secured Google Workspace environment under a signed Business Associate Agreement (BAA), meeting HIPAA standards for confidentiality and security. 

Other Information

• Paper Questionnaires can be used and entered manually in place of using a tablet or other device.

• The Questionnaire can be completed via smartphone for out of office appointments.
